Blended Service Coordinator
COMPANY OVERVIEW:
We are a community based social service agency that provides mental health case management services to Allegheny County’s highest risk population of children and adolescents between the ages of 3 and 21 years old. Our goal is to ensure that mental health services are delivered in the least restrictive environment, either by maintaining the consumer in the community or supporting the efforts to return the consumer back into the community. Our consumer specific services build on consumer, family, and community strengths. Case management services are provided with a creative and flexible approach to ensure quality services.
POSITION SUMMARY:
This individual will provide blended case management services to children and adolescents with mental illness who meet medical necessity criteria and need assistance in accessing, coordinating, and monitoring of resources and services. The Blended Case Manager will assess individual’s strengths and needs and will assist individuals/families in accessing resources and services that will build upon their strengths and needs in achieving stability in the community. This individual will perform this role in accordance will all established DPW, CCBHO, and AC MH/MR regulations, policies, and procedures and reports directly to the supervisor who provides general guidance regarding program issues.
QUALIFICATIONS:
Bachelor’s degree with major course work in sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, nursing, counseling or education with two years experience in the CASSP system. Strong interpersonal and organizational skills are a must.
DUTIES AND RESPONSIBILITIES:
1. Meets or exceeds the standard productivity of 1320 service units per quarter by the end of each quarter of the fiscal year.
2. Adheres to and maintains Gold Standards developed by CCBHO to continually improve the quality, consistency, and efficacy of Blended Case Management services.
3. Effectively and efficiently manages a caseload of 25 consumers as evidenced by adhering to all accessing standards, inputting and maintaining current records in the data base, turning all service docs in on a weekly basis, completing all service plans, crisis plans, and authorizations as per established time frames, and is present and prepared for weekly supervision.
4. Maintains client records in the Blended Case Management on-call book and follows all on-call procedures when on-call as assigned by the Program Director.
5. Complies with and adheres to all Blended Case Management policies and procedures including maintaining at least two (2) contacts monthly with client – one of which needs to be face-to-face.
6. Complies with and adheres to HIPAA confidentiality policy as evidenced by keeping all files locked, and not releasing any third party information and filing weekly.
7. Coordinates with families, providers, Children Youth and Families, juvenile probation, educational system and any other community resources as necessary to contribute to client’s success. Attendance at all ancillary meetings with systems involved (court hearings, permanency planning, IEP etc.).
8. Provide educational advocacy by attending any school meetings and providing technical assistance as needed.
9. Attends and participates in personal development and remains competent and knowledgeable of current trends in the mental health field as evidenced by maintaining at least 25 training hours per year.

ESSENTIAL REQUIREMENTS:
A. Possess Acts 33 and 34 clearances.
B. Have a valid Pennsylvania motor vehicle license and access to a personal vehicle.
C. Experience in CASSP system.
D. Strong oral and written communication skills.
E. Excellent leadership qualities.
F. Ability to work independently.
G. Full-time position requiring 38.75 hours per week and rotate on-call.
