EI Service Coordinator (Home Based)

 



  We are an Early Intervention agency in Midtown Manhattan serving children with developmental delays.  We are currently seeking a dynamic individual to take on the role of Service Coordinator.  This is an important position where you will learn diverse skills and have the ability to grow and advance.  As a Service Coordinator, you will have the opportunity to provide case management to families with infants and toddlers with developmental delays.  A key skill for this position will be the ability to build relationships with other agencies and Early Intervention Office Designees and most importantly, coordinate to meet the family’s needs. You will have the chance to work in a fast paced company where your input will be expected and appreciated. You will also have the flexibility of working at home and in the office. 



 

Responsibilities include:Guide families and help them understand what Early Intervention is and how it works.Provide case management to families with infants and toddlers with developmental delays. Plan and coordinate with families to receive evaluations and services. Develop plans and strategies to meet the family’s needs. Participate in the development, implementation, and monitoring of the Individualized Family Service Plan (IFSP). Ensure that approved services are provided in a timely manner. Maintain regular and ongoing contact with families. Monitor services and maintain quarterly progress reports and semi-annual IFSP reviews or amendments. Collaborate with our own case management department and outside provider agencies to secure needed services for families. Act as a liaison between therapists and family and EI providers, whenever necessary. Maintain proper documentation on families and children.

 

Requirements:Bachelor Degree in relevant field. i.e., Psychology, Social Work, Education etc. Experience in the Early Intervention ProgramFluent in both English and Spanish a plusSolid interpersonal communication - verbal and written - skills are required.Ability to multitaskTake initiative and be a decision makerStrong analytical skillsStrong typing skills and knowledge of MS Excel, Word and customized databasesSuccessful experience working in a high volume work environment where organizational and multi-tasking skills are essential.
