CLASS SUMMARY
The Developmental Disabilities Services Counselor works under supervision and
is responsible for determining client eligibility, obtaining client
information, preparing and monitoring case plans, coordinating services with
providers, and preparing information and recommendations. This position is
governed by state and federal laws and agency policy.
EXAMPLES OF WORK
l. Reviews referrals for services and requests for information to determine
eligibility or needs and provides information regarding Developmental
Disabilities Services (DDS), assistance, services, and other resources
available to requestor.
2. Schedules initial intake conference with client and/or family members
following eligibility determination to obtain written and oral information
regarding psychiatric, psychological, and medical background, financial
status, and previous professional recommendations.
3. Evaluates request and professional findings in accordance with DDS policy
and prepares case plan with recommendations and options for services,
referring client to alternative sources or preparing additional requests
for evaluation and assistance. Forwards requests to central office for
approval.
4. Contacts participating private and public service providers for
assistance, prepares contracts for services, and conducts case management
follow-up according to DDS policy.
5. Visits with client and family members in their homes to provide family,
parent, and behavior modification counseling and to ascertain needs.
6. Prepares weekly itinerary, reviews and updates caseload printout, prepares
changes to case plans, and forwards to central office.
7. Attends staffings to review problems, provide input into policy and
procedure review and development, and address community-based support
issues. Serves as a liaison to other divisions and agencies to develop
policies and procedures for community-based programs.
8. Develops and presents information regarding DDS and alternative services
to private and public organizations and workshops.
9. Performs related responsibilities as required or assigned.
WORKING RELATIONSHIPS
The Developmental Disabilities Services Counselor has regular contact with
clients, family members, departmental and other agency personnel, service
providers, and the general public.
SPECIAL JOB DIMENSIONS
May be required to work hours other than assigned shift in order to perform
job duties and responsibilities.
KNOWLEDGES, ABILITIES, AND SKILLS
Knowledge of the principles and practices of counseling.
Knowledge of community-based programs and services for the developmentally
disabled.
Ability to evaluate client information, assess client needs, and make
referrals.
Ability to prepare, present, and review oral and written information and
reports.
Ability to provide counseling and technical assistance.
MINIMUM QUALIFICATIONS
The formal education equivalent of a bachelor's degree in psychology,
sociology, or related field; plus two years' experience working with disabled
clients.
Other job related education and/or experience may be substituted for all or
part of these basic requirements upon approval of the Qualifications Review
Committee.
