Washington Center for Aging Services (WCAS) is a government owned 259 bed long-term care nursing facility, located in North East, Washington D.C.  The organization is managed under the direction of VMT Long Term Care Management Inc.

 

 

SCOPE OF POSITION

 

         Position is responsible for assisting in managing overall social services process for the department.

 

 

BRIEF DESCRIPTION OF DUTIES

 Perform individual social service work with residents and families members as requiredServe as an active participant on the Resident Council Advisory Board and Family Council Perform group work for various purposes using dynamics conceptParticipate in construction of resident care plans in conjunction with other interdisciplinary team membersComplete legally/regulatory required documentation for assigned case loadPerform necessary social services functions associated with the intake and discharge planning processFoster utilization of relevant community services based on resident needs and requestsFunction as a department liaison concerning the psychiatric consulting services of Medical StaffParticipate and facilitate research/projects/presentations as assignedServe as Lead Social Worker and assume additional responsibilities in absence of the Director or as designated

 

CORE COMPETENCIES

 Working knowledge of social services principles and practices as they apply to the long-term care setting in addition to adherence to applicable federal, local and industry specific regulations (i.e. CMS) governing the field of gerontologyExcellent leadership skills and ability to operate a a member of an interdisciplinary clinical team and function as Lead Social Worker to ensure task completion by required deadlinesSuperb clinical observation, assessment, and documentation skillsWorking knowledge of Medicaid/Medicare Ability to successful manage/document case load to meet licensure/inspection standards and handle casework/resident needs by applying psychotherapeutic approaches, assisting with financial matters, administering conflict resolution and utilize community resources to enhance services for resident populationInteract effectively with family members and provide consultative services to maximum relationship potential Ability to coordinate and facilitate discharge planning processAbility to conduct research, prepare findings and present teachings on relevant social services topicsAbility to interact professionally with various internal and external publicsAbility to research and disseminate information effectively orally and through written correspondence

 

MINIMUM QUALIFICATIONS

 Graduate of an accredited School of Social Work, Master’s required Registered Independent Clinical Social Worker or Clinical Social Worker with active District of Columbia license Demonstrated clinical, management, and assessment expertise with a minimum of one year of experience; preferably in a long-term care setting
