Nurse Home Visitor, NYC Nurse-Family Partnership – Helping First-Time Parents Succeed

The Nurse-Family Partnership (NFP) is a nurse home-visiting program for low-income, first time parents and their children, designed to improve maternal and child health outcomes, build secure and nurturing relationships between parents and children, and empower families to reach personal, educational and career goals. As a nurse home visitor, carrying a caseload of no more than 25 families, you will provide prenatal and early childhood case management to families until a child’s second birthday. During visits, you will assess and monitor the health of mothers and their infants, while empowering families to address issues varying from personal and environmental health, life course development, parental roles and care-giving, social support, to health and human services utilization.

NFP is a nationally replicated program that has been rigorously evaluated for over 25 years. The program has demonstrated significant improvements in maternal and child health outcomes, improved school readiness for children and families’ self-sufficiency. The New York City Department of Health and Mental Hygiene (NYC DOHMH) oversees all agencies implementing NFP in New York City. In 2003, the NYC DOHMH started its first site in Jamaica East, Queens and has since expanded to serve high-risk communities in all 5 boroughs: Queens (Jamaica East, Northwest Queens and the Rockaways), Bronx (North Central and South Bronx), Brooklyn (North and Central Brooklyn, East and West Brooklyn, Coney Island), Manhattan (East and Central Harlem, Washington Heights and Inwood, Lower Manhattan) and Staten Island (North Island). NYC DOHMH also operates a special NFP program for teens in foster care, women and teens in homeless shelters, and women at Rikers Island.
