Registered Nurse Consultant

Our Registered Nurses provide medical expertise, assistance and consultation for a client population that consists of frail, elderly, disabled and developmentally disabled adults.

 

RN Consultants provide medical expertise, input and consultation to a team of social work case managers whose clients are at high risk of residential placement or hospitalization due to unstable health conditions or caregiver/provider situations. May conduct home visits to assess the efficacy of client service plans, provide education and training of caregivers including disease process instruction and health promotion; and telephone contacts with physicians/home health workers, etc., to coordinate, monitor or reevaluate clients’ care and/or medical conditions. May also manage a small caseload of medically complex clients, including completing computerized, comprehensive psychosocial assessments.

 

Chronic Care RNs are responsible for clients being managed under our Chronic Care Program and will collaborate with ambulatory care teams in several King County clinic settings to design care delivery for patients with chronic illness, consistent with the Chronic Care Model. The Chronic Care RN will be a key member of the patient’s health care team in supporting patients as they develop self-management skills, and will help patients access and navigate appropriate healthcare and safety net systems. They will guide patients towards self-care through coaching, education, coordination and follow-up, perform home visits and pharmacological assessments, and assessments of environmental factors and/or other barriers that impact patients’ ability to access appropriate health care services. Must be able to respond to the changing needs of patients and clinics, and work in partnership with other members of the patient’s healthcare team.

 

Applicants must possess a valid WA State Registered Nurse’s license, a Bachelor's degree in Nursing and at two (2) years professional experience providing direct services to elderly/disabled persons.
