Home Health Services is a nonprofit, community-based home care agency that is accredited by JCAHO and certified by Medicare. Home Health Services has been meeting the needs for home health care specialized in serving the senior population since 1974.
We are currently seeking qualified candidates for Full Time RN Field Nurse position at our Dallas, TX office.
The full-time Field Admissions RN, is responsible for admitting patients to service, Recerts and Discharges, also provides skilled professional nursing care to the patient as prescribed by the physician and in compliance with regulations as established by the State Board of Registered Nursing, the patient, and the policies of the agency. The RN reports directly to an assigned Team Manager or Branch Manager.
Qualifications
· A Registered Nurse with current licensure to practice professional nursing in the State.
· Graduate of NLN accredited school of nursing with a BSN degree is preferred.
· Minimum of two (2) years experience as a professional nurse with some experience in Home Health Nursing, and supervisory experience.
· Must be able to perform wound care and IV functions, as well as other general nursing functions.
· Basic computer skills. Able to use MS Office packages and email.
· Complies with accepted professional standards and practice.
· Has excellent observation, good nursing judgment and communication skills.
· Is self directed with the ability to work with little supervision.
· Is flexible and cooperative in fulfilling all obligations. Able to accept constructive
criticism and use it in a positive manner.
· Has good interpersonal skills with understanding of supervisory management concepts.
Responsibilities and Duties
§ Provides professional nursing care by utilizing all elements to nursing process.
§ Assesses and evaluates patient’s status by:developing and initiating plan of care; regularly re-evaluating patient needs;
§ revising the plan of care as necessary.
§ Initiates and sustains the implementation of orders for medications and treatments as prescribed by the physician in the physician’s plan to care.
§ Initiates and applies appropriate preventative, therapeutic and rehabilitative nursing procedures and techniques and maintains communication regarding patient care with the physician and supervisory personnel.
§ Completes, maintains and submits timely, accurate, and relevant progress notes
regarding patient’s condition, care given, patient instructions, and response to teaching.
§ Uses supplies and equipment effectively and efficiently.
§ Observes the patient for changes in condition and reports these changes and the
patient’s needs to the attending physician and appropriate supervisory personnel.
§ Attends conferences with Team Manager for patient updates.
§ Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and independence.
§ Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet, and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers.
§ Assists the patient’s family/caregiver and other health care personnel in providing continuity of care.
§ Works in cooperation with the family/caregiver and home care team to meet the
physical, environmental and emotional needs of the patient and family/caregiver.
§ Supervises paraprofessional and HHA staff and delegates responsibilities when
required.
§ Acts as case coordinator on home care cases when accepting that responsibility.
Accepts responsibility of assignment to perform a specialized procedure, such as IV Therapy, only when qualified with specialty training, proven competency, clinical back-ground, and expertise in the specialty area.
§ Submits all required patient documentation according to agency timetables:time sheets and progress notes - every Monday;
o admission paperwork - within 24hours of initial visit;recert paperwork - 2 weeks prior to E.O.C.
o discharge and transfer summaries - 1 week after discharge/transfer.
Other Functions
§ Participates in the on call rotation.
§ Performs within the limits of preparation and experience.
§ Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and inservice classes and mandatory clinical staff meetings.
§ Fulfills the obligation of requested and/or accepted case assignments.
§ Participates in the quarterly record review audit as requested.
§ Functions as a team member to solve problems within the clinical department as well as other departments to improve overall function of the agency.
