Home Health Services is a nonprofit, community-based home care agency with six (6) branch locations in the Dallas-Fort Worth, TX area and due to recent expansion, a branch office in Houston, TX. Accredited by JCAHO and certified by Medicare. Home Health Services has been meeting the needs for home health care specialized in serving the senior population since 1974.

 We are currently seeking qualified candidates for a Full Time Physical Therapist (PT), Team Manager position for our Garland, TX office.

 

Our Physical Therapists are responsible for providing home health physical therapy services and for adherence to plans of care prescribed by a physician. In addition, the PT complies with the regulations and standards as established by the State Board of Physical Therapy and patient care policies and procedures of the agency.

 

 

Qualifications



·   Possesses a degree from a baccalaureate or Masters program in physical therapy approved by an accredited organization.

 

·   is licensed to practice as Physical Therapist in the state.

 

·   has one (1) year of appropriate experience as a Physical Therapist.

 

·   Physical Therapy experience in Home Health is a plus. 

 

 

Responsibilities and DutiesProvides physical therapy services to patient according to written physician's plan of care.

 Initiates physical therapy program and instructs other agency staff and/or family/caregiver in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the outcomes of the physical therapy program for the patient.

 Identifies patient and family/care giver needs for other home health services and refers as necessary.

 Assesses and evaluates patient's status

 Completes, maintains and submits timely and accurate progress notes
