AMBULATORY CASE MANAGER, RN

We are currently searching for an experienced Ambulatory Case Manager to join our growing Ambulatory Case Management Department.   In this collaborative position, you will be responsible for the assessment, planning, implementation, coordination and monitoring of services our members require to meet their individual needs using communication and our available resources to promote quality and cost-effective healthcare. 



Requirements
Current and active CA RN, CCM strongly desired
3-5 years prior experience in a similar role
Strong knowledge of community resources and programs
Able to work as a strong team member with a focus on excellent patient care
Able to communicate effectively with patients, family members and physicians to ensure effective patient care
Strong follow up skills
