Advanced Practice Nurse- Transitional Care- Part Time 20

The Advanced Practice Nurse will work with a multidisciplinary team to plan, develop and implement this new program that will begin to address the complex and varied needs of chronically ill patients within the hospital system who are reaching the age of adulthood and must seek healthcare services in the adult healthcare system. This APN will also be a consultant and resource to medical. Nursing and other staff to provide educations as well as develop processes and systems that ensure safe and effective transition of healthcare for adolescent patients. This opening is part time ( 20 hours per week). 1. Practices collaboratively with physicians and other health care providers to provide high quality health care to children. 2. Performs appropriate assessment of patient/family upon referral/admission. 3. Performs ongoing assessment and evaluation of patient progress and revision of interdisciplinary plan of care. 4. May use prescriptive authority if appropriately credentialed to do so. Utilizes appropriate medical staff for purposes of consultation and referral when necessary. 5. Identifies patient/family need for further resources and initiates appropriate referrals. 6. Assists the Staff Nurse in formulating the Nursing Care Plan and setting priorities based on the needs of patient and family. 7. Documents patient care and progress on patient's chart. 8. Coordinates Nursing Plan for Discharge in collaboration with the Staff Nurse and other members of the health care team. 9. Provides patient/family teaching based on identification of learning needs. 10. Evaluates compliance with the prescribed treatment regimen through telephone calls, family/patient assessments and outpatient visits as needed. A. Education, Training and Experience 1. Master of Science in Nursing 2. Three years of related clinical experience B. Licenses, Registrations, or Certifications 1. Current Registered Nurse license in the State of Delaware 2.
