Responsibilities: The Assessment Clinician is a qualified RN responsible for the patient assessments and collection of OASIS data. This individual is responsible for establishing an accurate and comprehensive physical and narrative assessment of the patient that thoroughly addresses and supports the clinical findings and the OASIS items selected, and the creation of a comprehensive care plan that addresses all patient needs. He/she functions under the direction and supervision of the Assistant Director of Clinical Services and/or Director of Clinical Services on his/her assigned care team.Qualifications: ~CB~ Date Posted 3/18/2008 1. A Registered Nurse with a Baccalaureate and two years clinical experience within the last three years in a home health agency, or a Registered Nurse with three years of clinical experience within the last five years in a home health agency. 2. Must meet educational and experience requirements outlined in the laws of licensure states. 3. Must Have Oasis certification. 4. Must have knowledge of Medicare and Medicaid requirements outlined in the laws of licensure states. 5. Must possess content knowledge of comprehensive patient assessments, and care planning, patient management and/or multi-discipline care management skills. 6.
