Asst. Director of Clinical Services

Overview: The ASSISTANT DIRECTOR OF CLINICAL SERVICES (ADCS) is a qualified professional responsible for directing a collaborative case management process which assesses, plans, implements, coordinates, monitors and evaluates options and services to meet the individual patient's health needs through communication and available resources to promote quality cost-effective outcomes.Responsibilities: This individual is responsible for the management and supervision of patients on his/her assigned care team, and for the care, treatments and services provided by the care team members.Qualifications: 1. A Registered Nurse with three years of clinical experience and at least one year of leadership experience within the last five years in a home health agency. A Baccalaureate or higher degree in nursing or other health related field and prior home care experienced required. 2. Professional licensure in the state(s) in which he/she is required to practice.
