Location: US-AZ-PHOENIX Status: Full Time, Employee Job Category: Medical/Health Reference Code: 51443 Job Description This position is accountable for leading the CHW CareConnect Performance Transformation activities in an assigned CHW hospital or region to support the successful implementation, adoption, and benefits realization of a comprehensive suite of clinical and business information systems. The Performance Transformation Lead is an integral part of the CareConnect team. The Lead provides innovation, leadership, and organizational development expertise related performance transformation activities of the organizational change, leader development, benefits realization and optimization of CareConnect. The Performance Transformation Lead is a registered nurse or related clinical or business professional with a strong knowledge base in the various clinical and business applications and has the ability to connect that with needed changes to integrate processes and work flow to afford best practices in patient care/treatment. This individual will work in synergy with various clinicians, physicians and business managers with the objective to promote stainable clinician and physician operational change and benefit realization. The Performance Transformation Lead will guide hospital leadership in the readiness, execution and engagement in process and workflow changes, coordinated with a successful and timely implementation of the CareConnect systems. This position leads the development of the CareConnect Performance Transformation plan for the sponsorship, standardization and organizational engagement of the CHW CareConnect model. This position is responsible for the transfer of clinical and business transformation knowledge, content and tools from the CHW CareConnect methodology to drive organizational readiness for change and to sustain positive change post go-live. Requirements: Clinical degree: Bachelors and/or Masters in Nursing or related clinical degree (i.e., Medical Technology, Radiology, Respiratory Therapy, Pharmacist, etc.) required. MBA, MHA, or equivalent in healthcare, information technology, or general business preferred.
