CASE MANAGER

 

The Case Manager directs and coordinates functions and activities of case management, utilization and discharge planning with the interdisciplinary team, involving both client and family in the decision making process, directs the use of health care services, and strives to improve quality of care and maintain cost effectiveness on a case-by-case basis. 

 

The ability to communicate effectively (written and verbal) with medical and administrative staff as well as patient and families, in an assertive, professional manner is essential.  Must be self motivated, self directed with excellent organization and time management skills.  Working knowledge of utilization management principals and TMF screening criteria is required, along with general knowledge of available community resources.   

 

Two years of experience in case management or utilization review and current Texas licensure as RN, LVN or Social Worker is required; one or more certifications in a specialty related to CM/CPUR obtained through a nationally recognized accreditation body are a plus.  Experience in insurance or commercial payer negotiation is also a plus.  Current BLS is required.
