Opportunity to support to People Living with HIV and AIDS at a health and human services organization located in downtown D.C.
EXPECTED JOB ATTENDANCE38 hours per week (M -10:00 a.m. – 7:00 p.m., T/W 9:00 a.m. – 6:00 p.m., and T/F. 9:00 a.m. – 5:00 p.m.)Salary Range $30,000 to $34,000 + health, dental, life insurance, and 403(b). SPECIFIC DUTIES
Work collaboratively with seasoned receptionist as back up support in managing frontline duties which include client In-Take scheduling, office traffic flow and in-coming calls. Meet with new clients to provide point of entry In-Take Assessments, as a member of the Case Management Team, for clients seeking Case Management, Primary Medical Care, Substance Abuse and Mental Health Treatment and other supportive services. Plan, coordinate, and implement case management services, to a partial case load of clients that include face-to-face and telephone contacts, bio-psychosocial assessments, care plans and reviews, and short and long-term interventions. Educate clients about services and entitlements available and make appropriate referrals, on-going collaborations with other service providers, and follow-up. Manage weekly/monthly schedules for case management counseling/interventions. Attend case management operating team meetings, weekly case review supervision, and multi-disciplinary case conference meetings with agency-wide team.
QUALIFICATIONS
Knowledgeable about HIV/AIDS issues and community resources and demonstrated cultural competence through prior work experience, education, and/or training. Bachelor degree in Social Work (BSW) required and 1 year of full-time Case Management experience. Additional years of CM experience welcomed. Spanish Speaker preferred. Significant prior professional experience in working closely with frontline reception staff in setting clear boundaries with clients, in a cordial, respectful, and professional manner; scheduling internal referrals by phone and in person; good oral and written communication and data base skills. Demonstrated ability to work effectively with diverse populations and groups.
