St. Agnes Continuing Care Center is seeking a full-time Case Manager for LIFE (Living Independently for Elders). The Case Manager will ensure effective clinical management/coordination of inpatient admissions/discharges. Additionally will serve as a liaison among LIFE physicians, interdisciplinary team (IDT), attending physicians and inpatient settings, such as hospitals, SNFs and nursing homes. Candidate must be a graduate of an accredited school of nursing with a current Pennsylvania RN licensure. Baccalaureate degree in nursing preferred. Minimum of two years experience in an acute care setting with medically complex adult participants.
