CLINICAL CASE MANAGER

Whittier Street Health Center's Public Health Initiative Program is searching for a Clinical Case Manager.

This position is responsible for monitoring and following –up with all chronic care patients, including those for diabetes patients, cancer screenings, patients without clinical visits in past six months, patients diagnosed with depression, asthma and obesity diagnosis etc. The Case Manager is also responsible for providing documentation in the patient record, assisting in planning and implementing programs and projects, and preparing reports and evaluations in addition to any other services that is needed.

Summary of Essential Responsibilities:

Plans and integrates care for people with chronic diseases.

Assists in planning and implementing programs and projects to improve quality and efficiency of educational and administrative services and operations.

Act as a community liaison for public health efforts towards healthier communities.

Coordinates services for persons with chronic condition and cancer screening services.

Develops education materials that promote positive healthy behaviors in populations identified to need assistance in targeted health related areas and communicates health information to various populations on- and off site;

Attends meetings on chronic disease management.

Ability/Experience/Skills:

Exceptional interpersonal, organizational, analytical, communication and customer service skills in working with multidisciplinary groups

The ability to work effectively with a diverse workforce and client base is essential

Should be flexible, demonstrate self-initiation and posses the ability to work independently yet also effectively participate as part of a team.

Knowledge of medical terminology
Experience in the community
Experience with working in multicultural setting
Strong communications and organizational skills
Excellent outreach skills
Strong computer skills

Qualifications:

An Associate Degree or higher with at least one year or more of related experience.

Bilingual in Spanish preferred, but not required.
