CLINICAL PHARMACIST - GREAT PLACE TO WORK

Title: Clinical/Staff Pharmacist  

Department: Inpatient Pharmacy

 

 

    General Summary

 

The Clinical/Staff Pharmacist is responsible for assuring the effective and efficient provision of pharmaceutical care services.  He/she must perform all duties necessary to provide for effective daily operation of departmental services and programs.  The pharmacist is proactively involved in safe, rational and cost-effective therapeutic decision making.  In the course of performing his/her duties, the pharmacist is responsible for the activities of pharmacy residents, students and supportive personnel as assigned in all practice areas.  The pharmacist reports directly to the immediate supervisor of the assigned practice area.

 

I. Qualifications

Must have a minimum of a Bachelor of Science degree in pharmacy and/or Pharm.D. and be licensed or eligible for licensure in Illinois. The incumbent must possess effective verbal, interpersonal and written communication skills.

 

II.  Duties and Responsibilities

 

1. Collects/obtains and evaluates physicians’ orders for proper disposition of the order, as well as therapeutic appropriateness, proper dosing, drug interactions, allergic reactions, route and rate of administration and therapeutic redundance.

2.  Clarifies physicians’ orders when necessary through direct and timely contact with prescriber or designated practitioner.

3. Enters physicians’ orders or supervises technician order entry into the patient record in the pharmacy computer system. Documents allergies, diagnoses, weights and ages where appropriate.

4.  Performs daily review activities, (e.g., minibag audit, drug interaction monitoring, diagnosis and allergy review).

5. Provides general supervision for technical support in their assigned area.

6. Monitors and coordinates workflow with established delivery schedules.

7.  Provides patient specific drug information as required to health professionals.

  8.   Participates in drug use evaluation (DUE) as assigned.

 

III. Knowledge, Skills and Activities

 



1.  Maintains working knowledge of medical center and departmental policies and procedures, including safety issues, inventory control and distribution, and infection control.

2.  Maintains currency in the professional and clinical literature of the profession.

3.  Demonstrates a basic understanding of clinical monitoring activities.

4.  Demonstrates the ability to use standard texts and other sources of information and apply it to general patient care questions.

5.  Effectively communicates with pharmacists, nurses and physicians.



 

IV. Other Activities

1. Assists in maintaining required quality assurance records.

2. Responds to the department’s Disaster Plan as prescribed by hospital policy.

  3. Reviews policies and procedures when requested.

 

 

QUALIFICATIONS:Minimum Requirements·  Graduate of an accredited school of pharmacy

·  Licensed and in good standing with the Board of Pharmacy

·  Experience with and knowledge of sterile IV admixture procedures

·  Excellent communication skills, flexibility and ability to work as part of a multi-disciplinary team

·  Self-directed with excellent time management skills and able to work independently

·  Current valid driver’s license and proof of current automobile insurance
