General Summary:
In this role you will have the opportunity to join a newly created department called our Disease Management department. You will be responsible for coordinating services between members and their physicians that may be suffering from chronic conditions. You will also have the opportunity to educate patients on the appropriate use of the health care system based on clinical judgment, established guidelines and client needs. You must be willing to support the Call Center to assure consistent service delivery during the hours of operation.

Job Requirements include:
Registered Nurse with current RN license in good standing requiredExperience in disease or case managementPrevious (5 years) clinical RN experience in one of the following clinical areas: telephone triage, ER, ICU, CCU or Urgent CareExcellent verbal and written communication skillsComputer experience preferredAbility to work independently or in a team environment
