Cardiac Program Nurse   Facility: Caritas Home Care / Norwood Office, Norwood MA Department: Home Care - Full-time - 8:00 am-4:30 pm, every 4th wknd. - Day shift - Licensure Required - 1-2 years acute care experience is required - Basic computer skills required - Customer service skills are required - Home Care experience is preferred

 

 Focus of this role is Cardiovascular disease care and COPD. Provides nursing care to cardiac and pulmonary patients and families in their homes as prescribed by a physician and consistent with Home Care department policy and procedures, the Heart Care Program Protocols, the COPD Program Protocols and Telemedicine policies and procedures. Will also provide nursing care to home care patients and families with other diagnoses. This is accomplished within an interdisciplinary plan of care. Works collaboratively with all team members, and office support staff to coordinate patient care utilizing community resources as needed. Provides consultation regarding assessment, planning and implementation of care; also instructs nursing personnel in the care of select patients with cardiac and pulmonary disease. Participates in orienting, supervising and evaluating Home Health Aides. Monitors telemedicine data. Participates in staff education of cardiac, pulmonary and telemedicine patient care. Coordinates activities with other community, agency and/or department staff and carries out special activities on assignment.
