As a Care Coordination Nurse / Patient Advocate, you would enhance the quality and efficiency of healthcare and promote well being to our members. This individual will contact members telephonically to assess health needs and refer them to appropriate services. Primary responsibilities include: Identifying members with unmet healthcare needs who may be at risk for hospital admission or readmission Assessing and managing readmission risk related to targeted chronic/serious diseases Communicating with members and providers via telephone to assess the quality and appropriateness of care after a patient is discharged from the hospital Collaborating with other clinical staff to enhance the quality and efficiency of healthcare delivery Documenting work done on each case Participating in weekly meetings with a medical director to review casesThe chosen candidate will possess: A current RN license in the state of Florida Demonstrated acute clinical experienceAdditional assets include: Clinical experience in a variety of settings Certified Case Manager status Managed care, home healthcare and/or care management experience Coronary Artery Disease experience Fluency in Spanish (strongly preferred) Computer proficiencyUnitedHealth Group offers a full range of comprehensive benefits to full-time and part-time employees, including medical, dental and vision, as well as a matching 401k and an employee stock purchase plan.
