The Care Coordinator collaborates with the interdisciplinary team to assess and develop a plan to meet the complex needs of patients in order to achieve optimal functioning and timely discharge. The Care Coordinator monitors and support the implementation of the plan which may include coordination of referral to community-based services. The Care Coordinator also supports patients and their families to meet psychological needs relating to crisis, trauma, chronic illness, end of life issues, and substance abuse.

BSN required.
MSN preferred.
Minimum continuing education of 30 hours pre year in relevant required.
3-5 years recent acute care experience- preferred
Current AZ RN license- required.
BLS (American Heart Association)- required upon hire.
National certification in relevant specialty within two years of hire-preferred.
