Care Manager Openings (LCSW, LMFT, RN or Psychologist) Salary + Benefits

Holman was first on the West Coast and one of the first nationally to pick up the responsibility for managing the relationship between benefit dollars spent and outcome.

We have two openings for full-time Care Managers.

License Requirements: LCSW, LMFT, RN, or Psychologist.

Qualifications: Demonstrated experience in mental health or chemical dependency is required. Experience in the following is preferred: case management, DSM IV diagnosis, assessment, consultation, intervention, along with good verbal and written communication skills.

Care Manager duties to include: Care managing mental health cases in order to ensure the delivery of continuous quality care, including assistance with crisis calls.

• Facilitates the service authorization process for both inpatient and outpatient care.
• Coordinating of care plan to assist patients in achieving the best possible outcomes.
• Conducts telephonic reviews.
• Consuls with physicians when needed.
• Conducts review of care utilizing established criteria.
• Consults with providers, beneficiaries and family members as needed.
• Documents all initial and concurrent reviews, including, but not limited to diagnosis, symptoms, interventions, goals and treatment plan.
