The hired Care Manager will determine the medical needs of high-acuity Medicare and Medicaid members. Key responsibilities include: Conducting Health Risk Assessments (HRA) to determine member needs Coordinating care and getting appropriate authorizations Responding to incoming calls from assigned members Documenting all cases in an internal database systemTo be considered for this position, you must be a licensed RN in Hawaii with significant clinical experience and good customer service skills.
