Case Management Department Manager

SeniorsPlus has an immediate opening for the position of Case Management Department Manager in our Elder Independence of Maine (EIM) division.  This position manages the Auburn and Bangor Consumer Services department by coordinating the overall delivery of telephonic case management for consumers of home-based care services and ensures development of care plans for comprehensive case management.This position supervises 5 team leaders, and as part of our leadership team, implements policies & procedures, assesses compliance & performance goals, participates in quality enhancement activities, and represents EIM in a multi-system environment.  The ideal candidate must be an MSWor RN with a minimum of four years experience in a management position requiring supervision and knowledge of Long Term Care programs in Maine. Clinical knowledge of home health care for elderly and disabled adults preferred. S/He must be a strong leader with excellent communication, organizational, and interpersonal skills, and have a commitment to a team environment.
