BASIC FUNCTION:This position is responsible in a clinical advisory role to assess members for wellness education and disease management. Will introduce members to the Personal Health Manager website as a tool to better manage their health. Will educate members regarding wellness and specific conditions; facilitate coordination of care for members identified through claims. Will be responsible for completion of Health Risk Assessments and self-referral/ referral from the UM and/or CM departments, or physician referrals. . JOB REQUIREMENTS:1) Current, unrestricted OK licensed Registered Nurse. 2) Three years clinical experience. 3) Recent patient education experience. 4) Computer/system software experience including Web. 5) Verbal and written communication skills; analytical skills; sound clinical judgment.PREFERRED JOB REQUIREMENTS:1) Disease Management experience. 2) Recent patient teaching experience in asthma, diabetes, coronary artery disease, COPD, and Congestive Heart Failure
