Case Manager - Full Time Day Shift - RN Req'd, 2 yrs related experience req'd

Vibra Specialty Hospital of Dallas is expanding its staff.  We are currently seeking a Case Manager to join our team.  The Case Manager has primary accountability for the development, coordination, and management of each patient's treatment plan to a successful outcome.  He/she works diligently with the payor source, patient and team to develop and deliver measurable quality outcomes.  He/she also has accountability for appropriate sequencing of treatment goals, the delivery of efficient/cost efficient care, and the planning and implementation of a comprehensive discharge plan.  The Case Manager is a key member of the facility's management and marketing teams and works with Admissions to facilitate patient admissions.

Qualifications and Experience

RN required; BSN preferred. 
A minimum of 2 years case management and/or hospital discharge planning experience required.
Must have excellent clinical skills as well as the ability to communicate well verbally and in writing. Click Here to Apply Online
