Program Overview:
CHL's HOAP Case Management Program provides assistance to chronically homeless individuals with HIV/AIDS, mental illness, and /or addiction diagnoses and who are working to transition to permanent housing. The program, using a "Housing First" model, utilizes a multi-disciplinary team of service providers who work to link program participants to a complement of services that includes screenings, assessments, counseling, health care, rehabilitation, and referral to other support systems. Staff also work with individuals to help them enroll in available health insurance programs and social security programs.
Job Description:
FT (37.5 hrs/wk) Case Manager to:Provide supportive services to clients who are participating in one of the HOAP Supportive Housing Programs (i.e., HIV Supportive Housing, Shelter + Care, Homelink), Transitional Housing, or other permanent housing programs.Assist clients with the acquisition of safe, appropriate housing resources. Work with area landlords and property managers to assist clients with maintaining their tenancy and understanding the goals and responsibilities of preserving their housing. Understand HUD eligibility requirements for the HOAP Housing Program and perform appropriate rent calculations. Convene bi-annual ISP (Individual Service Plan) meetings with housing clients and providers. Assist clients in establishing and identifying steps to increase independence, reach self sufficiency (i.e. recovery from addiction, recovery from mental illness, maintaining optimal physical health, securing employment, etc.) Assist clients with activities of daily living (ADL's), budgeting and other services as needed.Assist clients with transportation for clinical appointments and/or ISP related activities. Prepare and maintain confidential client documentation in the Service Point (ETO) system. Coordinate and collaborate with other providers as needed. Assist clients with maintaining various entitlements and benefits (i.e., SSI, EAEDC, MassHealth). Perform outreach to clients in their natural environment as planned in ISP and as needed.Attend relevant case management training activities. Identify gaps in homeless services and provide data for the purpose of client advocacy. Prepare and maintain confidential client documentation, service records, staff activity logs (SALS) and preparation of statistical reports. Regularly attend weekly HOAP Staff meetings, clinical team, supervision and all other required program meetings.Perform all other duties as required.
Qualifications:Bachelor's degree required; degree and/or coursework in social work, counseling, psychology or related field preferred. Minimum two years of experience in field required Current valid US-issued driver's license and ability to provide registered and insured automobile for work related purposes Desire to serve others required. Demonstrated leadership ability and/or potential required. Experience in social services and/or working with program's population preferred. Bilingual (Spanish/English) preferred
