Program Overview:

CHL's Community Rehabilitation Services program, which operates at our Lipton Center facility is a behavioral health clinic providing continuing care services. 90% of thos who utilize the clinic's services are DMH-referred; the clinic sees a very diverse spectrum of diagnoses including personality disorders as well as dual and multi -diagnoses. The small clinic setting utilizes a multi-disciplinary team approach led by our on-site psychiatrist. Clinicians work in a very challenging, supportive and learning environment with significant opportunities for professional development.

 

Job Description:

 

FT (37.5 hrs/wk) Case Manager to: engage clients in treatment.provide transportation to appointments.provide support in the community.monitor treatment compliance.participate in interdisciplinary meetings.interact with other care providers.

Qualifications:Bachelor's degree required; degree and/or coursework in social work, counseling, psychology or related field preferred. Current valid US-issued driver's license and ability to provide registered and insured automobile for work related purposes Desire to serve others required. Demonstrated leadership ability and/or potential required. Experience in social services and/or working with program's population preferred.
