Caseworker I - Roanoke, VA

 

First Home Care provides services that enable youth, families and adults to receive the care they need in their homes and communities, reducing the need for hospitalization and out-of-community placement. Our programs offer free-standing, independent levels of care; however, they also function as contracted, wraparound services. As such, they provide ancillary treatment to support programs of social service agencies, special education programs, and office-based therapies.

We are curently seeking a Caseworker I to join our team in Roanoke, VA. 

Caseworkers provide overall case management of children and adolescents referred for foster care services. Support and provide consultation to therapeutic foster care parents, families of children and adolescents receiving services and other members of the treatment team. Coordinates case management activities including planning, placement monitoring, crisis intervention and discharge planning. Works in the home of therapeutic foster care parents and families of clients in the community. Position requires frequent local travel. Schedule will vary and will include evenings, nights and weekend work.

Position requires a Bachelor's degree Social Work, Human Services or related field and one year of experience in case management working with adolescents and/or children and families. Must posses a valid driver's license.
