Clinical Care Coordinator

Job Description:
In conjunction with physicians and members of the breast health team, the Clinical Care Coordinator serves as a clinical expert in the delivery of nursing care to the breast health patient. Responsibilities include working in a collaborative and supportive manner with the breast radiologist, primary care physicians, breast surgeons, specialists and diagnostic and treating facilities to facilitate the patient through the process system; providing advance patient care directly as a case manager and assistant during all image guided biopsies, providing pre and post assessment and education as well as emotional support and indirectly as an educator, counselor, consultant, coordinator and researcher. Co-facilitate the breast center support groups as necessary. Must have the ability to provide care to patients from adult through geriatric years. The Clinical Care Coordinator will service as a knowledgeable individual who will afford breast health patients services, facilities and equipment that may not be readily available or accessible without assistance or intervention on their behalf. Must be able to multi task and work in a fast paced environment.

Qualifications:
Registered Nurse licensed by the State of GA. BCLS Specialty Specific Certification Preferred. Membership in Professional Organization strongly recommended.

Shift Information:
FT Monday - Friday.
