Position Description

 

Date: November 1, 2007    Reports to: Director of Managed Care

Position: Clinical Liaison

FLSA: Exempt 

 

Position Purpose:

 

Under the direction of the Director of Managed Care, the Liaison will represent the TCCS facilities at the hospitals to facilitate seamless transfers of the patients from the hospitals to TCCS skilled nursing facilities.  This role is crucial for the development of census in the TCCS communities.

 

General Responsibilities will Include:Representing the TCCS communities in the hospitals.Understanding of admissions process to the Skilled Nursing Facilities.Collaboration with Discharge Planning Staff at Acute Care Hospitals.Collaboration with patient and or family regarding potential admission to skilled nursing facility.Review of Patient medical records, and or assessment of patient for appropriate level of care.Understanding of skilled nursing regulations.Completes and records comprehensive and accurate pre admission assessment identifying medical, social and financial issues, and communicating them effectively to the communities.Coordination of actual discharge.Hospital Rounds on potential admissions and on patients who have been transferred from our facility to the hospital, and reports progress to facility.Responsible for obtaining medication history from hospital for 7 day look back period.

 

Minimum Qualifications:

·  RN licensed in the State of Michigan

·  Excellent communication skills

·  Experience as a liaison, LTC nurse or case manager.

·  Must be comfortable speaking/negotiating with discharge planners and Insurance case Managers.

·  Understanding of various insurance methodologies, including Medicare, Medicaid, and Managed Care insurance.

·  Requires considerable logic, ability to analyze problems and determine a course of action,

·  Requires tact, insight and leadership skills.

·  Candidates should have excellent communication and presentation skills with the ability to communicate with multiple levels in the organization.

·  Candidates just be able to work in a collaborative and shared leadership environment.

·  Must possess and demonstrate personal presence that is characterized by a sense of honesty, integrity and caring with the ability to inspire and motivate others to promote the philosophy, mission, vision, goals, and values of Trinity Health.

 

Physical and Mental requirements and working conditions:

Requires the ability to concentrate, meet deadlines and adapt to frequent interruptions.Excellent organizational and communicative skills are necessary.Ability to walk long distances daily, occasional lifting carrying and bending.
