Clinical Social Worker

About the Opportunity

As a social worker for the company, you will be an important member of our team of professionals.  The primary responsibilities include making phone contact with patients who are seriously ill or their primary caretakers, assessing their health & financial status/needs, determining what resources are available to them in their area and assisting with securing those resources.  This is an office-based position with weekday hours in an easily accessible inner loop location.

 

Qualifications

Prefer at least 3 years of clinical social work experience (LCSW or LMSW)

Excellent written and verbal communication skills

Ability to organize and prioritize work responsibilities

Ability to function independently with minimal supervision

Bilingual (Spanish) preferred

Hospital or community clinic experience helpful

Knowledge of Medicaid, Medicare, SSA processes and criteria helpful
