Essential Job Functions:Performs the assessment function for all adult clients and derives nursing diagnoses based on assessment data. Plans nursing interventions in collaboration with the interdisciplinary team to resolve current problems and prevent potential problems.  Begins to establish the discharge plan at time of admission and includes the patient/caregiver in planning process.  Progressively tapers visits prior to discharge in order to support a smooth transition out of home care.  Involves the patient/family in determining care goals and objectives.  Acts as a patient advocate. Implements nursing interventions to maximize health status and minimize symptoms associated with acute or chronic disease.  Delivers care within the scope of nursing practice and in accordance with accepted standards of practice.  Demonstrates and instructs care to be provided by the patient, family, or caregiver and provides ongoing supervision and instructing as warranted.  Provides emotional support to patients and families in a manner that fosters independence, affirms existing and new strengths, and promotes and develops strengths.  Respects the autonomy and right to confidentiality of all patients and families.Identifies need for intervention by other members of the interdisciplinary team on an ongoing basis; collaborates in the planning of such care.  Reports changes in patient condition to the attending physician and seeks needed changes in pharmacological intervention or plan of care, as warranted.  Documents effectiveness in response to the plan of care. Recognizes elements of patient and family systems that are not amenable to change. Identifies the need for extension of home health services, provides complete and accurate supporting documentation, and seeks physician order to recertify the patient.  Assesses the need for home health aide services.  Plans, teaches and supervises the provision of such services.  Provides on-site orientation and supervision of Home Health Aides on an ongoing basis.  Documents and reports to appropriate supervisor any identified performance issues..Participates in the orientation of new members of the interdisciplinary team.  As requested, makes joint observation visits with residents, VNA employees, community agency representatives and students.Identifies and articulates essential components of home health benefits as administered under Medicare and other payors and demonstrates an understanding of home health reimbursement methodologies to manage a caseload and deliver care in a cost-effective manner.

 



Requirements:Associate’s degree in Nursing.  Bachelor’s degree in Nursing preferred.Current unrestricted license to practice as a registered nurse.One year of current home health care or hospital experience preferred.BCLS certification.
