CSG, a leading provider of mental health, mental retardation, children’s and eldercare services has full time Community Support Advisor opportunities in our CONCEPTS Day Habilitation and Support Services programs.   

CONCEPTS’ Day Habilitation Services provide individuals with the skills and support necessary to acquire, retain and improve self-help, socialization and adaptive skills. Our habilitation services provide daily or weekly support to the individual in a day services setting and in the community.





Job Description



 

The Community Support Advisor is responsible for developing and facilitating individualized programs of functional skills instruction to promote independence, interdependence, and community integration.  This instruction is individually scheduled and can be implemented in a day services environment and/or community settings.

 

Job Duties:

 Assesses and documents instructional needs initially and on an ongoing basis.Develops and facilitates skills instruction in appropriate settingDocuments program information and other contacts as appropriateCompletes all assigned administrative tasks as appropriateCollaborates and cooperates with involved agencies, family members, and residential providers; participates in meetings and provides relevant information.Utilizes community resources to promote community integration and client independence and interdependenceDrive 15 passenger vanPerforms other duties as assigned



 Job Requirements



 

Must be 18 years of age and have either a high school diploma or GED certification.

Must possess good communication skills; possess a valid driver’s license with a safe driving record. Must have a reliable vehicle.

 

 

Condition of Employment

 

Must be able to pass driving and criminal history background checks.

 

Desirable Experience



 

Experience working with individuals with individuals with disabilities or pursuing a degree in human services preferred
