Community Support Worker - CPRC

Preferred Family Healthcare is seeking a full time Community Support Worker for our Trenton, Missouri location.  The incumbent of this position is responsible for providing community services for individuals with mental illness and/or substance abuse problems onsite and/or within the community.

 

Essential Job Functions: 



 Complete Community Support assessment/Initial Discharge Planning Session and identify treatment issues.  Complete regular communication with referral sources/guardians regarding progress, transition planning, and pertinent clinical issues.Request correspondence from past treatment.Participate in staffing to assure of continuity of care.Make or assist in outside referral of issues not able to be addressed in treatment.Assist in scheduling of treatment and arranging transportation.  Initiate effort to contact and attempt to re-engage through notifying consumer/guardian/referral source of any absence within 48 hours, as noted in DMH Standards.Responsible for the continuous development and evolution of treatment plans in an individualized manner.Represent the agency in a professional manner.Ensuring or assisting in the referral for medical issues of clients.Linking families with services in the community.  Advocate on behalf of the family.Assisting client/family in transitional planning.Correct any chart deficiencies within a timely manner.Document all services provided in accordance with DMH/CARF Standards.Provide crisis intervention as necessary.  Adhere to weekly direct service expectations.Facilitate group education as scheduled.Obtain trainings to assist in professional development obtaining 36 hours within 2 year timeframe.Other duties as assigned by the Clinical Supervisor, Program Director, Vice President, or Senior Executive Team member.







Knowledge, Skills, and Abilities: 

·        

·         Knowledge of case management and rehabilitation methods, principles and techniques as they relate to mental health and/or substance abuse treatment services.

·         Knowledge of the problems encountered by persons with mental illness and/or substance abuse disorders and how to access the various community resources available.

·         Knowledge of the methods and modalities as well as the legal requirements and court procedures related to treating mental illnesses and substance abuse.

·         Some knowledge of supervisory and training techniques and practices used in a substance abuse/mental health setting.

·         Aptitude to evaluate services within assigned program and give input.

·         Capable of communicating ideas clearly and effectively verbally and in writing to produce clear and concise reports.

 







Experience and Education Qualifications:Graduate from an accredited college or university with a Bachelor’s Degree in social work, counseling, psychology or closely related field; OR

 Has at least four (4) years relevant, qualifying full-time equivalent experience in human service delivery and demonstrates skills in developing positive and productive client relationships and the ability to negotiate complex service systems to obtain needed services and resources for clients; OR

 For CPRC:  LPN plus two years Human Service experience, AND

 All CSTAR case management staff shall successfully complete a department approved CSTAR case management training course within six (6) months of employment as a case manager.
