Continuity of Care Service Director - Kaiser Vallejo

Kaiser Permanente is an AA/EEO employer. DEPARTMENT: Continuity of Care SCHEDULE: Full – Time Regular; 40 hours per week; Day Shift; Monday through Friday, 9:00am to 5:00pm. EDUCATION: BSN or equivalent Bachelor’s degree CERTIFICATION/LICENSE: CA RN license POSITION SUMMARY: This role reports directly to the Chief Operating Officer, with a dotted line to the Assistant Administrator for Patient Care Services. The COCSD is responsible for Resource Management, Discharge Planning and Social Services. The COCSD works closely with administrative and physician leaders to support patient flow within the hospital, appropriate utilization of hospital inpatient resources and appropriate disposition of patients post discharge from acute care. Has responsibility for above areas for Napa Solano, which includes 2 medical centers. QUALIFICATIONS: Proven experience in management of staff, inpatient case management, resource management, project management. Required Knowledge of TJC standards, Nurse Practice Act, NASW Code of Ethics and other local, state, and federal regulations. Previous supervisory experience required (usually 4 years). Demonstrated strong interpersonal communication, problem solving, conflict resolution and team building skills. DUTIES: Manages the operations of 24 hr. Continuing Care Department that may be at multiple locations. This includes 24 hour coverage by Discharge Planning RN’s in the ED. Through their managers, oversees functions of Discharge Planning and Social Services. In concert with other key stakeholders, develops and monitors the Resource Management strategic plan for hospital utilization. Works closely with the services of the Continuum to assure appropriate placement of patients following hospitalization. In collaboration with Nursing Services, assures that regulatory standards are met for their staff and services provided by their staff. Collects, maintains and monitors data to facilitate review of opportunities for efficiency. Develops programs that make hospital systems more efficient. Works with Patient Business Services to assure systems are in place to capture revenue from non-KP sources. Serves as a content expert in teaching management and physicians about utilization and reimbursement opportunities. With the Chief of Utilization Management chairs the UM Committee and the Traction Committee. Takes call for nursing related issues in rotation with other Service Directors. Responsible for continuous readiness related to meeting TJC standards for Discharge Planning and Social Services. Provides data for review of physician provider effectiveness related to inpatient UM goals. Ensures compliance with all HR tracking and documentation requirements. Determines appropriate staff mix for department and develops/implements processes to screen, interview, hire, train and maintain competency of all department staff. Ensures ongoing staff development. Develops and monitors budgets for appropriate use of human and material resources. Monitors financial performance and identifies and implements strategies to reduce costs and improve quality of care and service. Directs department quality improvement activities. Reports out on quality and utilization data as requested.Manages and resolves human resources, employee, department safety and risk management issues.Consistently supports compliance and the Principles of Responsibility (Kaiser Permanente's Code of Conduct) by maintaining the privacy and confidentiality of information, protecting the assets of the organization, acting with ethics and integrity, reporting noncompliance, and adhering to applicable federal, state and local laws and regulations, accreditation and licenser requirements (if applicable), and Kaiser Permanente's policies and procedures.
