Diabetes Educator, RN

- Bachelors degree is required

- 3-5 years experience is required

Teaches patients and their families or significant others about diagnosed illness, providing them with information and teaching skills to learn to manage their condition. Evaluates learning experience and outcome methodology. Provides community health education. Assesses patient's and/or family's ability to learn, identifies barriers, and prioritizes patients' learning needs. Uses stages of change methodology with appropriate interventions. Develops a comprehensive teaching/learning plan for the patient and family. Implements plan then reassesses intervention and evaluates outcome. Collaborates with PCP and/or Endocrinologist regarding patients' diabetes management skills, motivation and behavior.

BSN with a minimum of 4 years clinical experience and current New Jersey RN license required. Certified Diabetes Educator preferred or must after meeting the 1000 hours of direct diabetes education, must sit for the exam by the NCBDE within 8 months of completing the hours. HJ
