Director, Chronic Care Improvement Program

NAMM California, a leader in the Managed Care industry, has an immediate opportunity for the right candidate as Director of Chronic Care Improvement Programs.  This position is located at our Rancho Cucamonga, California location.  The position responsibilities and requirements are as follows:         Will provide leadership and administrative oversight, program development, implementation, and evaluation of chronic care programs and support IPA medical management functionProvides direct day to day supervision of the NAMM Case Management staff and utilization of general resourcesParticipates in determination of department’s goals and objectives, setting organizational structure and staffingKnowledge of Medical Management and IPA operationsStrong computer skills data analysis skillsMSN/MS preferredBSN required or equivalent experience in Case Management, Disease Management or Chronic Care leadershipCurrent California R.N. LicenseValid Drivers License and insuranceCCM Certificate preferred
