Director of Admissions//Clinical Case Manager

RN Required

 

Exciting innovative opportunity for a registered nurse to follow the resident from

admission to discharge.  Responsible for the admission of new residents and  coordination

of resources to facilitate delivery of care; as well as for community outreach and the promotion

of the facility’s services to targeted agencies, groups, and organizations.

 

Requirements include: minimum of three years’ experience in a long-term care setting,

knowledge of admissions/community outreach, case mix, Medicare/Medicaid regulations,

bachelor’s degree preferred in Nursing or related field.

 

Basic knowledge should include:

MA nursing home screening and admitting regulations, payment sources, as well as computer

literacy.
