Director of Social Services

Eastcastle Place, Inc, a retirement community setting a new standard in senior living on Milwaukee's eastside, offering a full continuum of care including independent living, assisted living, rehabilitation, skilled nursing, and memory support.

The Director of Social Services is in charge for providing social services to the residents of Eastcastle Place, Inc. Candidate will be responsible for maintaining appropriate Social Services records & information files, assist in admissions in coordination with RN MDS Admissions Coordinator, coordinating discharge planning & post discharge follow-up, meeting with residents and families to help adjust to facility and attend to issues, investigate any allegations of abuse, neglect, or misappropriation of funds, and completing the Social Service assessment. Knowledge of MDS and Medicare guidelines preferred.

Qualified candidates would have a Bachelors degree in social work and at least 3 years of experience in a long term care facility. Eastcastle Place, Inc. provides a competitive wage and benefit package.
