LandStone Recruitment has two opportunities for compassionate, patient focused individuals to make a difference, Director of Social Work and a Social Worker. This family owned and operated Long Term Care facility is ready to build their team in order to provide the well rounded services and case management that their residents deserve. Director of Social ServicesIn this role you will need to have at least 2 years experience as a Director in long term care. Please do not apply if you do not have long term care experience as a Director. Social WorkerThis is a part time role and we again prefer a candidate with long term care experience. You can also email your resume and cover letter to kneal@landmarkhealth.com General Statement of Job Function: This position will assist with admissions, discharges and care planning process for residents assigned by Director. The daily interaction with resident is to help meet their emotional and psychosocial needs. Some interaction with family and significant others is also a valued part of this role. Minimum Qualifications: This position prefers at least a Bachelor prepared candidate with experience in long term care. Current Social Work license in the state in which facility is located. Persons who have been found guilty by a court of law of abusing, neglecting, or mistreating individuals are ineligible for employment in the position. Employee will not consume or be under the influence of alcohol or other controlled substances. Knowledge of regulatory standards and compliance requirements. Working knowledge of personal computer and software applications used in job functions helpful. Ability to read, write and speak English on a daily basis is required. Organizational skills and ability to prioritize needs also required. Working knowledge and ability to apply professional standards of social work practice in job situations. Ability to define problems: collects data, establish facts, and draw valid conclusions. Then proceed to resolution consensus. Job Function, Responsibilities, and Expectations: Meet with the resident and family to review policies on advanced directives, resident rights and discrimination as part of admission process and annually as needed. Demonstrate the ability to meet with residents, family and significant others to assist with care planning, financial planning, psychosocial interventions, and general communications regarding room changes and/or change of condition. Demonstrate ability to document these communications listed above and show ability to follow through on outstanding issues with resident, family, or interdisciplinary team. Keeps a current list of community resources for residents and families who are seeking assistance not provided in the facility. Is able to articulate the facility mission and philosophy and communicate the same to co-workers, residents, family and community appropriately. Knowledge of emergency and disaster procedures of facility. Demonstrates respect for residents, families, staff and community. Developing working relationships that provide opportunities for effective communication of information. Ability to comply with facility policies and procedures for workplace safety for self and others. Demonstrates ability to become an active part of the Care Team in facility and is able to participate in process improvement to monitor the quality of care provided to residents. Access continuing education opportunities appropriate to responsibilities. Participates in orientation of new staff and ongoing education of current staff to better meet the residents needs and/or define and reinforce the process by which ongoing improvements can be made. Attends in-service and education programs; attends continuing education required for maintenance of professional certification or licensure. Work Environment: The Social worker will need to be able to lift up to 25 pounds and carry items 100 yards. They will need to be able to walk the facility several times a day and participate in emergency situations. The ability to see, hear, write and speak English on an ongoing basis is a daily necessity of job.
