Discharge Planner, Social Worker

(EOE/M/F/V/D) Requirements Must have BSW, MSW or LCSW in Social Work and proper licensure. Must have a degree from an accredited college. Two years of healthcare social work preferred. Must be very organized with good communication skills. Medical knowledge preferred.
