As a Disease Management Support Coordinator, you would work closely with the nurses to ensure that members' needs are addressed and that they understand their conditions and required treatments. Apply today to join our fun, supportive team. Primary responsibilities include: Working with the nurse in CCS/F (Care Coordination System/Frame), to address member needs as identified by RN, check eligibility status and look up providers Making and responding to calls to assist members with enrollment needs and other issues (60% of each day) Following cases on an ongoing basis to assist nurses with non-clinical work Faxing information to doctor's officesRequired qualifications include: Experience providing customer service A professional phone presence and excellent communication skills Attention to detail, flexibility, strong organizational skills and the ability to multitask Word and Excel proficiencyPreferred qualifications include: Knowledge of medical terminology A background involving Medicaid and Medicare Proficiency with IP Agent phone system/CCS/F and VCC (Virtual Call Center) UnitedHealth Group offers a full range of comprehensive benefits, including medical, dental and vision, as well as a matching 401k and an employee stock purchase plan.  UnitedHealth Group is a drug-free workplace. Candidates are required to pass a drug test before beginning employment. Diversity creates a healthier atmosphere:  equal opportunity employer M/F/D/V.
