Llanfair House Nursing and Rehabilitation Center, a 180 bed sub-acute and long term care facility, located in Wayne, NJ is currently seeking a full time External Case Manager Responsibilities include, assessing patients for possible admission, promoting Llanfair House to patients, family members and hospital Case Managers, working with the hospital discharge team to ensure a smooth transition to continuing care. Travel to area hospitals is required. Valid NJ RN license is required. Case Management or Long Term Care experience preferred.
