Monarch HealthCare is currently searching for service dedicated Geriatric Nurse Practitioner to join our growing Clinical Services Department.

 

In this newly created position, you will be responsible for providing assessment and disease management skills in conjunction with the management of all patients in skilled care facilities, homebound services and long-term care centers.  In addition, you will assist the Medical Team with identification, evaluation and coordination of all health care services provided to the Monarch HealthCare population of patients with high risk, intensive care needs for heart failure management. Work collaboratively with the Hospitalist Physicians with post-hospitialization planning and case management.

 

The successful candidate will posses the following attributes: 

 A current and active CA RN/RNP with no restrictionsAble to perform medical acts and approved to do so by the joint Board of Medical Examiners and Board of Registered NursingWork in collaboration with both internal and external departments, families and membersStrong communication skills both oral and written and be able to communicate complex medical issues to non-clinical family members3-5 years prior experience in a similar role at a hospital, community organization, health plan or IPA
