At Silverado At Home, we serve our clients by giving life through meaningful engagement in the setting of their choice. We are currently seeking a Geriatric Care Manager to join our growing care team in the Los Angeles area!


The Geriatric Care Manager directs the care plan and interventions designed for specific individuals and families. This effort may include assessment, counseling, coordination of needed services, assigning and ensuring quality delivery of care-giving within home settings, and assistance with placement if indicated.


Ideal candidate background will include:

- Either a Master’s Degree in Social Work, Gerontology, or related field or a Registered Nurse Degree with experience in Geriatric Care Management strongly preferred.

- Several years of experience in care management, home care, long-term care or other senior-service related field is preferred.

- A successful track record indicating a working knowledge of clinical geriatric care issues, family dynamics and interventions, education and training, supervision, quality assurance and customer service is desirable.
