Graduate Nurse, RN - Cardiac Care

Requirements: This position is open to Graduate Nurses. May 2008 graduates encouraged to apply.Graduate from a state accredited school of nursing with current Wisconsin licensure or Graduate Nurse awaiting licensure. Flexibility in scheduling to meet the twenty four (24) hour needs of patients. Possess organizational, problem solving and critical thinking skills. Effective communication skills with patients, families and the interdisciplinary team. Able to maintain stable performance level under conditions of pressure and multiple demands. Maintains current health Care Provider Basic Life Support certification. Meets all unit specific competencies within the specified time frame. Description: We are currently inviting May 2008 graduate nurses to join our Cardiac Care team. The Patient Care Coordinator(PCC)is an RN or graduate nurse who is responsible to provide care in accordance with the American Nurses Association Standards of Practice and other standards established by professional groups, regulating bodies and agencies. Nursing care is delivered in accordance the the Wheaton Fransican Healthcare-All Saints Standards of Care. The PCC is responsible for the provision and coordination of nursing care to include the collection and analysis of assessment data in determining nursing diagnosis, identification of expected outcomes and development of plan of care in collaboration with the patient/family and other interdisciplinary team members. The PCC identifies interventions to attain expected outcomes, implements the interventions identified in the plan of care, evaluates and documents progress toward attainment of outcomes. This care includes sensitivity to age specific and unique needs of the patient/family. The PCC promotes and evaluates the effective functioning of the health care team, collaborates with team members to meet the identified needs of the patient/family throughout the episode of care and delegates tasks and activities as appropriate.This position will work in our Cardiac Care Units. This is a full-time position working 12 hour night shifts.
