Health Educator

Job Description: I. TITLE: Health Information Providers and Practice Flow Facilitators II. SCOPE: Health Information Providers and Practice Flow Facilitators provide service to patients under the direction of the Practice Director at the Mount Sinai Adolescent Health Center and/or his/her designee. Health Information Providers and Practice Flow Facilitators will be assigned to individual patients with whom they will remain in contact as needed throughout current and subsequent clinic encounters. Health Information Providers and Practice Flow Facilitators will accompany patients through the clinic, assisting with instructions, directions, obtaining appointments, and general access to services. In addition, facilitators will respond to patient telephone inquiries including specific adolescent relevant health education information. III. RESPONSIBLE TO: The Mount Sinai Adolescent Health Center Practice Director or designee IV. PRINCIPLE DUTIES AND RESPONSIBILITIES: Health Information Providers and Practice Flow Facilitators will function within the Mount Sinai Adolescent Health Center. He/she must be HIPPA trained and must carry out all responsibilities under HIPPA guidelines Duties and Responsibilities 1. Deliver walk-through orientation to AHC services for new patients 2. Address reasons for patient's visit and connect patient to appropriate service 3. Connect patients to providers, nurses, technicians, registrars, health enrollment counselors, etc. and facilitate the flow between these services 4. Assist in getting test results and relevant health information to the provider and patient as needed 5. Give visit flow progress status updates to both patients and providers 6. Develop liaison with Health Educators and attend related supervision sessions and trainings7. Counsel adolescents on family planning and other reproductive health/healthcare topics including pregnancy prevention and STI risk reduction as indicated8. Answer patient phone inquiries and provide education and direction for obtaining services9. Arrange specialty and follow-up appointments in coordination with patient's insurance coverage and track visit reports from outside facilitiesThe Purpose of the Position 1. To provide timely live response to patient phone inquiries Miscellaneous Duties 1. Patient liaison2. Patient satisfaction surveillance as indicated3. Participate in center wide quality assurance4. Participate in evaluation process Mount Sinai Adolescent Health Center Practice Flow Facilitators must comply with approved dress code and must wear hospital ID at all times in the unit.When approaching a patient or staff member, the Practice Flow Facilitator must introducehim/herself and identify their role on the unit.Job Qualifications: Requirements and Qualifications:College degree in related fields preferred. (ie: business administration, health education, communications, psychology, counseling, etc). Work Experience in similar positions will also be considered.
