Home Care Dietitian - Per diem

Effectively utilizes knowledge of dietetics in providing diet instruction for clients and their families. Develops good working relationships with health team personnel. Responsible for accurate diet instruction for clients. Effectively uses personal time and department resources. Evaluates client's nutritional needs as related to medical and social-economic status, home and family resources. Identifies client's dietary needs and develops long and short-goals to meet those needs. Applies dietary procedures to achieve identified goals. Observes records and reports to the physician/dentist and agency personnel the client's reaction to treatment and any changes in the client's condition. Functions in accordance with a diet prescribed by the physician. Instructs other health team personnel and family members. Participates in the planning and development of staff inservice education programs. Professional Registered Dietitian (RD) with a bachelors or advanced degree from an accredited institution. Completion of a dietetic internship or similar hospital or public health experience is mandatory.
